{"id":6222,"date":"2017-05-18T05:17:59","date_gmt":"2017-05-18T01:17:59","guid":{"rendered":"http:\/\/aroxjblog.am\/?p=6222"},"modified":"2017-05-18T05:18:45","modified_gmt":"2017-05-18T01:18:45","slug":"%d6%84%d5%b6%d5%ab-%d6%85%d5%a2%d5%bd%d5%bf%d6%80%d5%b8%d6%82%d5%af%d5%bf%d5%ab%d5%be-%d5%a1%d5%ba%d5%b6%d5%b8%d5%a7","status":"publish","type":"post","link":"https:\/\/aroxjblog.am\/en\/%d6%84%d5%b6%d5%ab-%d6%85%d5%a2%d5%bd%d5%bf%d6%80%d5%b8%d6%82%d5%af%d5%bf%d5%ab%d5%be-%d5%a1%d5%ba%d5%b6%d5%b8%d5%a7\/","title":{"rendered":"Sleep apnea"},"content":{"rendered":"<p><b>Sleep apnea<\/b>, also spelled <b>sleep apnoea<\/b>, is a <a title=\"Sleep disorder\" href=\"https:\/\/en.wikipedia.org\/wiki\/Sleep_disorder\">sleep disorder<\/a> characterized by <a title=\"Apnea\" href=\"https:\/\/en.wikipedia.org\/wiki\/Apnea\">pauses in breathing<\/a> or periods of <a title=\"Hypopnea\" href=\"https:\/\/en.wikipedia.org\/wiki\/Hypopnea\">shallow breathing<\/a> during <a title=\"Sleep\" href=\"https:\/\/en.wikipedia.org\/wiki\/Sleep\">sleep<\/a>.\u00a0Each pause can last for a few seconds to several minutes and they happen many times a night.\u00a0In the most common form, this follows loud <a title=\"Snoring\" href=\"https:\/\/en.wikipedia.org\/wiki\/Snoring\">snoring<\/a>.\u00a0There may be a choking or snorting sound as breathing resumes. As it disrupts normal sleep, those affected are often sleepy or tired during the day.\u00a0In children it may cause problems in school or <a class=\"mw-redirect\" title=\"Hyperactivity\" href=\"https:\/\/en.wikipedia.org\/wiki\/Hyperactivity\">hyperactivity<\/a>.<\/p>\n<p>There are three forms of sleep apnea: <a title=\"Obstructive sleep apnea\" href=\"https:\/\/en.wikipedia.org\/wiki\/Obstructive_sleep_apnea\">obstructive<\/a> (OSA), <a title=\"Central sleep apnea\" href=\"https:\/\/en.wikipedia.org\/wiki\/Central_sleep_apnea\">central<\/a> (CSA), and a combination of the two called mixed. OSA is the most common form.\u00a0Risk factors for OSA include being <a title=\"Overweight\" href=\"https:\/\/en.wikipedia.org\/wiki\/Overweight\">overweight<\/a>, a family history of the condition, <a class=\"mw-redirect\" title=\"Allergies\" href=\"https:\/\/en.wikipedia.org\/wiki\/Allergies\">allergies<\/a>, and enlarged <a class=\"mw-redirect\" title=\"Tonsils\" href=\"https:\/\/en.wikipedia.org\/wiki\/Tonsils\">tonsils<\/a>.\u00a0In OSA, breathing is interrupted by a blockage of airflow, while in CSA breathing stops due to a lack of effort to breathe. People with sleep apnea are often not aware they have it. Often it is first observed by a family member.\u00a0Sleep apnea is often diagnosed with an overnight <a title=\"Polysomnography\" href=\"https:\/\/en.wikipedia.org\/wiki\/Polysomnography\">sleep study<\/a>. For a diagnosis of sleep apnea, more than five episodes an hour must occur.<\/p>\n<p>Treatment may include lifestyle changes, mouthpieces, breathing devices, and surgery. Lifestyle changes may include avoiding <a title=\"Alcohol\" href=\"https:\/\/en.wikipedia.org\/wiki\/Alcohol\">alcohol<\/a>, weight loss, stopping smoking, and sleeping on their side. Breathing devices include the use of a <a class=\"mw-redirect\" title=\"CPAP machine\" href=\"https:\/\/en.wikipedia.org\/wiki\/CPAP_machine\">CPAP machine<\/a>. Without treatment sleep apnea may increase the risk of <a title=\"Myocardial infarction\" href=\"https:\/\/en.wikipedia.org\/wiki\/Myocardial_infarction\">heart attack<\/a>, <a title=\"Stroke\" href=\"https:\/\/en.wikipedia.org\/wiki\/Stroke\">stroke<\/a>, <a class=\"mw-redirect\" title=\"Diabetes\" href=\"https:\/\/en.wikipedia.org\/wiki\/Diabetes\">diabetes<\/a>, <a title=\"Heart failure\" href=\"https:\/\/en.wikipedia.org\/wiki\/Heart_failure\">heart failure<\/a>, <a class=\"mw-redirect\" title=\"Irregular heartbeat\" href=\"https:\/\/en.wikipedia.org\/wiki\/Irregular_heartbeat\">irregular heartbeat<\/a>, <a title=\"Obesity\" href=\"https:\/\/en.wikipedia.org\/wiki\/Obesity\">obesity<\/a>, and <a class=\"mw-redirect\" title=\"Motor vehicle collisions\" href=\"https:\/\/en.wikipedia.org\/wiki\/Motor_vehicle_collisions\">motor vehicle collisions<\/a>.<\/p>\n<p>OSA affects 1-6% of adults and 2% of children.\u00a0It affects males about twice as often as females.While people at any age can be affected it occurs most commonly among those 55 to 60 years old.Central sleep apnea affects less than one percent of people.A type of central sleep apnea was described in the German <a title=\"Myth\" href=\"https:\/\/en.wikipedia.org\/wiki\/Myth\">myth<\/a> of <a class=\"mw-redirect\" title=\"Ondine's curse\" href=\"https:\/\/en.wikipedia.org\/wiki\/Ondine%27s_curse\">Ondine&#8217;s curse<\/a> where when asleep the person would forget to breathe.<\/p>\n<h2><span id=\"Signs_and_symptoms\" class=\"mw-headline\">Signs and symptoms<\/span><\/h2>\n<p>People with sleep apnea have problems with <a title=\"Excessive daytime sleepiness\" href=\"https:\/\/en.wikipedia.org\/wiki\/Excessive_daytime_sleepiness\">excessive daytime sleepiness<\/a> (EDS), impaired alertness, and vision problems. OSA may increase risk for driving accidents and work-related accidents. If OSA is not treated, people are at increased risk of other health problems, such as diabetes. Even death could occur from untreated OSA due to lack of oxygen to the body.\u00a0Moreover, people are examined using &#8220;standard test batteries&#8221; in order to further identify parts of the brain that may be adversely affected by sleep apnea,\u00a0including those that govern:<\/p>\n<ul>\n<li>&#8220;executive functioning&#8221;, the way the person plans and initiates tasks<\/li>\n<li>paying attention, working effectively and processing information when in a waking state<\/li>\n<li>using memory and learning.<\/li>\n<\/ul>\n<p>Due to the disruption in daytime cognitive state, behavioral effects may also be present. These can include moodiness, belligerence, as well as a decrease in attentiveness and energy.Another symptom related to sleep apnea is <a title=\"Sleep paralysis\" href=\"https:\/\/en.wikipedia.org\/wiki\/Sleep_paralysis\">sleep paralysis<\/a>, the fear of which can sometimes lead to <a title=\"Insomnia\" href=\"https:\/\/en.wikipedia.org\/wiki\/Insomnia\">insomnia<\/a>. These effects may become intractable, leading to depression.<\/p>\n<p>There is also evidence that the risk of diabetes among those with moderate or severe sleep apnea is higher.\u00a0There is also increasing evidence that sleep apnea may also lead to liver function impairment, particularly fatty liver diseases (see <a title=\"Steatosis\" href=\"https:\/\/en.wikipedia.org\/wiki\/Steatosis\">steatosis<\/a>).\u00a0Finally, because there are many factors that could lead to some of the effects previously listed, some people are not aware that they have sleep apnea and are either misdiagnosed, or just ignore the symptoms altogether.<\/p>\n<h2><span id=\"Risk_factors\" class=\"mw-headline\">Risk factors<\/span><\/h2>\n<p>Sleep apnea can affect people regardless of sex, race, or age. However, risk factors include:<\/p>\n<ul>\n<li>being male<\/li>\n<li>excessive weight<\/li>\n<li>an age above 40<\/li>\n<li>large neck size (greater than 16\u201317 inches)<\/li>\n<li>enlarged tonsils or tongue<\/li>\n<li>small jaw bone<\/li>\n<li>gastroesophageal reflux<\/li>\n<li>allergies<\/li>\n<li>sinus problems<\/li>\n<li>a family history of sleep apnea<\/li>\n<li>deviated <a title=\"Nasal septum\" href=\"https:\/\/en.wikipedia.org\/wiki\/Nasal_septum\">septum<\/a><\/li>\n<\/ul>\n<p>Alcohol, sedatives and tranquilizers may also promote sleep apnea by relaxing throat muscles. Smokers have sleep apnea at three times the rate of people who have never smoked.<\/p>\n<p>Central sleep apnea is more often associated with any of the following risk factors:<\/p>\n<ul>\n<li>being male<\/li>\n<li>being older than 65 years<\/li>\n<li>having heart disorders such as <a title=\"Atrial fibrillation\" href=\"https:\/\/en.wikipedia.org\/wiki\/Atrial_fibrillation\">atrial fibrillation<\/a> or <a title=\"Atrial septal defect\" href=\"https:\/\/en.wikipedia.org\/wiki\/Atrial_septal_defect#Patent_foramen_ovale\">atrial septal defects such as PFO<\/a><\/li>\n<li>stroke<\/li>\n<\/ul>\n<p><a title=\"Hypertension\" href=\"https:\/\/en.wikipedia.org\/wiki\/Hypertension\">High blood pressure<\/a> is very common in people with sleep apnea.<\/p>\n<h2><span id=\"Mechanism\" class=\"mw-headline\">Mechanism<\/span><\/h2>\n<p>When breathing is paused, carbon dioxide builds up in the bloodstream. Chemoreceptors in the blood stream note the high carbon dioxide levels. The brain is signaled to wake the person sleeping and breathe in air. Breathing normally will restore oxygen levels and the person will fall asleep again.<\/p>\n<h2><span id=\"Diagnosis\" class=\"mw-headline\">Diagnosis<\/span><\/h2>\n<p>Sleep apnea may be diagnosed by the evaluation of symptoms, risk factors and observation, (e.g., excessive daytime sleepiness and fatigue) but the gold standard for diagnosis is a formal sleep study (<a title=\"Polysomnography\" href=\"https:\/\/en.wikipedia.org\/wiki\/Polysomnography\">polysomnography<\/a>, or sometimes reduced channels home based test). A study can establish reliable indices of the disorder, derived from the number and type of event per hour of sleep (Apnea Hypopnea Index (<a class=\"mw-redirect\" title=\"Apnea-hypopnea index\" href=\"https:\/\/en.wikipedia.org\/wiki\/Apnea-hypopnea_index\">AHI<\/a>), or Respiratory Disturbance Index (<a class=\"mw-redirect\" title=\"Respiratory Disturbance Index\" href=\"https:\/\/en.wikipedia.org\/wiki\/Respiratory_Disturbance_Index\">RDI<\/a>)), associated to a formal threshold, above which a patient is considered as suffering from sleep apnea, and the severity of their sleep apnea can then be quantified. Mild OSA (Obstructive Sleep Apneas) ranges from 5 to 14.9 events per hour, moderate OSA falls in the range of 15\u201329.9 events per hour, and severe OSA would be a patient having over 30 events per hour.<\/p>\n<p>Despite this medical consensus, the variety of apneic events (e.g., hypopnea vs apnea, central vs obstructive), the variability of patients&#8217; physiologies, and the inherent shortcomings and variability of equipment and methods, this field is subject to debate.\u00a0Within this context, the definition of an event depends on several factors (e.g., patient&#8217;s age) and account for this variability through a multi-criteria decision rule described in several, sometimes conflicting, guidelines.\u00a0One example of a commonly adopted definition of an apnea (for an adult) includes a minimum 10 second interval between breaths, with either a neurological arousal (a 3-second or greater shift in <a class=\"mw-redirect\" title=\"EEG\" href=\"https:\/\/en.wikipedia.org\/wiki\/EEG\">EEG<\/a> frequency, measured at C3, C4, O1, or O2) or a <a title=\"Blood\" href=\"https:\/\/en.wikipedia.org\/wiki\/Blood\">blood<\/a> <a title=\"Oxygen\" href=\"https:\/\/en.wikipedia.org\/wiki\/Oxygen\">oxygen<\/a> desaturation of 3\u20134% or greater, or both arousal and desaturation.<\/p>\n<h3><span id=\"Oximetry\" class=\"mw-headline\">Oximetry<\/span><\/h3>\n<p><a class=\"mw-redirect\" title=\"Oximetry\" href=\"https:\/\/en.wikipedia.org\/wiki\/Oximetry\">Oximetry<\/a>, which may be performed over one or several nights in a patient&#8217;s home, is a simpler, but less reliable or accurate alternative to a formal sleep study (<a title=\"Polysomnography\" href=\"https:\/\/en.wikipedia.org\/wiki\/Polysomnography\">polysomnography<\/a>). In one study, normal overnight oximetry was highly <a title=\"Sensitivity and specificity\" href=\"https:\/\/en.wikipedia.org\/wiki\/Sensitivity_and_specificity\">sensitive<\/a> and so if normal, sleep apnea was considered unlikely.\u00a0In addition, home oximetry may be effective in guiding prescription for automatically self-adjusting <a title=\"Continuous positive airway pressure\" href=\"https:\/\/en.wikipedia.org\/wiki\/Continuous_positive_airway_pressure\">continuous positive airway pressure<\/a>.<\/p>\n<h2><span id=\"Classification\" class=\"mw-headline\">Classification<\/span><\/h2>\n<p>There are three types of sleep apnea. The elderly are more likely to have OSA than young people. Men are more likely to suffer sleep apnea than women and children are, though it is not uncommon in the last two population groups.Obstructive sleep apnea (OSA) is the most common category of sleep-disordered breathing. The muscle tone of the body ordinarily relaxes during sleep, and at the level of the throat the human airway is composed of collapsible walls of soft tissue which can obstruct breathing. Mild occasional sleep apnea, such as many people experience during an <a class=\"mw-redirect\" title=\"Upper respiratory infection\" href=\"https:\/\/en.wikipedia.org\/wiki\/Upper_respiratory_infection\">upper respiratory infection<\/a>, may not be significant, but chronic severe obstructive sleep apnea requires treatment to prevent low blood oxygen (<a title=\"Hypoxemia\" href=\"https:\/\/en.wikipedia.org\/wiki\/Hypoxemia\">hypoxemia<\/a>), <a title=\"Sleep deprivation\" href=\"https:\/\/en.wikipedia.org\/wiki\/Sleep_deprivation\">sleep deprivation<\/a>, and other complications.<\/p>\n<p>The risk of OSA rises with increasing body weight, active smoking and age. In addition, patients with diabetes or &#8220;borderline&#8221; diabetes have up to three times the risk of having OSA.<\/p>\n<p>Common symptoms include loud <a title=\"Snoring\" href=\"https:\/\/en.wikipedia.org\/wiki\/Snoring\">snoring<\/a>, restless sleep, and sleepiness during the daytime. Diagnostic tests include home <a class=\"mw-redirect\" title=\"Oximetry\" href=\"https:\/\/en.wikipedia.org\/wiki\/Oximetry\">oximetry<\/a> or <a title=\"Polysomnography\" href=\"https:\/\/en.wikipedia.org\/wiki\/Polysomnography\">polysomnography<\/a> in a sleep clinic.<\/p>\n<p>Some treatments involve lifestyle changes, such as avoiding alcohol or muscle relaxants, losing weight, and quitting smoking. Many people benefit from sleeping at a 30-degree elevation of the upper body or higher, as if in a <a title=\"Recliner\" href=\"https:\/\/en.wikipedia.org\/wiki\/Recliner\">recliner<\/a>. Doing so helps prevent the gravitational collapse of the airway. <a class=\"mw-redirect\" title=\"Human anatomical terms\" href=\"https:\/\/en.wikipedia.org\/wiki\/Human_anatomical_terms#Anatomical_directions\">Lateral positions<\/a> (sleeping on a side), as opposed to <a title=\"Supine position\" href=\"https:\/\/en.wikipedia.org\/wiki\/Supine_position\">supine positions<\/a> (sleeping on the back), are also recommended as a treatment for sleep apnea,\u00a0largely because the gravitational component is smaller in the lateral position. Some people benefit from various kinds of oral appliances such as the <a title=\"Mandibular advancement splint\" href=\"https:\/\/en.wikipedia.org\/wiki\/Mandibular_advancement_splint\">Mandibular advancement splint<\/a> to keep the airway open during sleep. <a title=\"Continuous positive airway pressure\" href=\"https:\/\/en.wikipedia.org\/wiki\/Continuous_positive_airway_pressure\">Continuous positive airway pressure<\/a> (CPAP) is the most effective treatment for severe obstructive sleep apnea but oral appliances are considered a first line approach equal to CPAP for mild to moderate sleep apnea according to the AASM parameters of care.\u00a0There are also surgical procedures to remove and tighten tissue and widen the airway.<\/p>\n<p>Snoring is a common finding in people with this syndrome. Snoring is the turbulent sound of air moving through the back of the mouth, nose, and throat. Although not everyone who snores is experiencing difficulty breathing, snoring in combination with other risk factors has been found to be highly predictive of OSA.\u00a0The loudness of the snoring is not indicative of the severity of obstruction, however. If the upper airways are tremendously obstructed, there may not be enough air movement to make much sound. Even the loudest snoring does not mean that an individual has sleep apnea syndrome. The sign that is most suggestive of sleep apneas occurs when snoring <i>stops<\/i>.<\/p>\n<p>Other indicators include (but are not limited to): <a class=\"mw-redirect\" title=\"Hypersomnolence\" href=\"https:\/\/en.wikipedia.org\/wiki\/Hypersomnolence\">hypersomnolence<\/a>, obesity BMI &gt;30, large neck circumference (16\u00a0in (410\u00a0mm) in women, 17\u00a0in (430\u00a0mm) in men), enlarged tonsils and large tongue volume, <a class=\"mw-redirect\" title=\"Micrognathia\" href=\"https:\/\/en.wikipedia.org\/wiki\/Micrognathia\">micrognathia<\/a>, morning headaches, irritability\/mood-swings\/depression, learning and\/or memory difficulties, and sexual dysfunction.<\/p>\n<p>The term &#8220;sleep-disordered breathing&#8221; is commonly used in the U.S. to describe the full range of breathing problems during sleep in which not enough air reaches the lungs (<a title=\"Hypopnea\" href=\"https:\/\/en.wikipedia.org\/wiki\/Hypopnea\">hypopnea<\/a> and apnea).
